
#AI-171 Boeing 787 Air Crash : Initial Findings Published…A preliminary investigation report, released by the Aircraft Accident Investigation Bureau (AAIB) of India, gives us the first clear idea of what caused the tragedy. According to the report, the main cause was fuel supply to both engines being accidentally cut off just seconds after takeoff. Without fuel, the engines shut down, and the plane had no power to stay in the air.

On June 25, 2025, Air India Flight AI-171, a Boeing 787 Dreamliner, took off from Ahmedabad airport around 1:39 PM, heading to London Gatwick. Sadly, within just a minute of takeoff, the aircraft crashed into a hostel building at B.J. Medical College in Ahmedabad. The crash killed over 200 people, including passengers, crew, and people on the ground. Only one passenger survived.

Critical failure midair
The Fuel Control Switches, which regulate the flow of fuel to the engines, had been moved from the “RUN” to the “CUTOFF” position—effectively shutting down both engines mid-air. While the flight crew attempted to restart the engines, they were unable to do so before the aircraft lost altitude and crashed.
Investigators confirmed that the plane reached a maximum altitude of only 625 feet before losing thrust. The Cockpit Voice Recorder (CVR) captured a brief but alarming exchange between the pilots as they tried to understand the sudden loss of power. One pilot was heard asking, “Why did you cut off the fuel supply?” with the other responding, “I did not.”

The crew attempted to restart the engines, but there wasn’t enough time or altitude to do so successfully. The plane lost all thrust and crashed about 36 seconds after becoming airborne.

A mechanical fault or human error?
The aircraft was 11years old and had recently undergone maintenance. No prior issues were flagged.

The AAIB is examining whether the fuel cutoff was caused by Pilot error (accidental switch movement), a technical malfunction, or a possible design flaw in the Boeing 787’s fuel control system.

Aviation Experts involved
In addition to India’s AAIB, officials from the U.S. National Transportation Safety Board (NTSB), UK Air Accidents Investigation Branch (AAIB-UK), and Boeing are actively participating in the investigation. The black box data, recovered and analyzed in late June, has been critical in understanding the sequence of events.

This marks the first-ever fatal accident involving a Boeing 787 Dreamliner, a model known for its modern technology and safety record. The findings have triggered renewed scrutiny of cockpit procedures and aircraft system design. India’s Directorate General of Civil Aviation (DGCA) has ordered safety audits of Boeing 787 fleets operating in India, and some Dreamliners were briefly grounded for checks.
